Volunteers Needed for Commencement, Memories on the Mall |
Staff members interested in volunteering at the Fall 2018 Commencement Ceremonies and Memories on the Mall are invited to sign up by Friday, Dec. 7. Commencement duties include serving as greeters, ushers or shuttle guides. Duties for Memories on the Mall include distributing alumni tote bags to graduates and serving refreshments.
Available shifts include:
Tuesday, Dec. 18
Commencement:
- 7:45-10:30 a.m.
- 10:15 a.m.-1 p.m.
- 12:45-3:30 p.m.
- 3:15-6 p.m.
- All Day (7:45 a.m.-6 p.m.)
Memories on the Mall
- 9:45-11 a.m.
- 12:15-1:30 p.m.
- 2:45-4 p.m.
- 5:15-6:30 p.m.

Online Consent to Receive Form 1095-C
In accordance with the Affordable Care Act (ACA) provisions, UT Dallas will have the 2018 Form 1095-C available on or before Jan. 31, 2019. The Form 1095-C includes information about the health insurance coverage offered to employees, retirees and their dependents. You will receive the Form 1095-C if you were enrolled in the UT Medical Plan, if you worked an average of 30 or more hours per week or were considered a full-time employee in 2018.
If you provide consent, you can access your Form 1095-C online through Galaxy. If you provided consent in 2017, you do not have to do so again. The deadline to provide online consent is Jan. 13, 2019. If you do not provide the online consent, you will receive the form in the mail on or before the IRS deadline. The forms are expected to be available online on or before Jan. 31, 2019.
If you need to provide online consent, log in to Galaxy with your UT Dallas NetID and password. Then, go to Staff Tools, select ACA Form 1095-C and follow the on-screen instructions. Written instructions and FAQs are posted on the HR website for your reference.

Mammogram Event Coming Up
The UT Southwestern Mobile Mammography Unit will be on campus from 8 a.m. to 3 p.m., Wednesday, Nov. 28, Lot T (north of Residence Hall North). UT Dallas faculty and staff members, as well as their spouses, can schedule mammogram appointments by calling 214-645-2560 or 866-277-0710. Have your health insurance information available when you call. Sponsored mammograms are also available for patients without health care coverage.

$1,500 Delivered; Food Drive Continues
Members of the Office of Human Resources hand a check for $1,500 to Comet Cupboard staff. The money is the prize UT Dallas won earlier this year during MyEvive’s “Ace Your Benefits” challenge.
A month-long food drive continues this month. HR is leading the effort to collect cereal, peanut butter and jelly for the Comet Cupboard. The goal is to collect 1,200 items.
Please drop off items in the HR collection bins in AD 2.208 and SPN 1.127. If you have a large donation, email Carol Kidwell or call ext. 5327.

Callier Center Offers Discount for Staff, Faculty
The Callier Center for Communication Disorders is dedicated to helping people with hearing loss including faculty, staff and students.
By showing your Comet Card at either Callier’s Richardson or Dallas locations, you can receive 10 percent off premium-level hearing devices, custom hearing protective devices and/or assistive listening devices.
In addition, the UT Dallas community can visit the Callier Hearing Health Center, where audiologists can demonstrate technology and devices, while providing education on hearing health.

Students, staff and faculty are invited to attend CommuniTEA: Immigration and Feminism from noon-1 p.m., Tuesday, Nov 27, in the Galerstein Gender Center (SSB 4.300). Learn about the state of immigration today, how it affects women and children and why feminist efforts are vital to the advancement of immigration reform. RSVP today.

Experts to Share Insights at AIDS Day Luncheon
The Student Wellness Center and Galerstein Gender Center’s World AIDS Day Luncheon, to be held from 11 a.m. to 1 p.m., Thursday, Nov. 29 (SSA 14.265), will feature a panel of experts that will dispel the myths about HIV/AIDS and provide information on this issue.
Lunch will be provided, but you must RSVP in advance. Attendees are encouraged to “go red” and wear red clothing items in support.

McDermott Library Events
RefWorks: Introduction and the Basics, 11 a.m.-noon, Tuesday, Nov. 27 (MC 2.524): This web-based citation management tool is used to import and organize citations, and generate bibliographies or reference lists. The workshop covers creating an account, exporting citations from research databases, managing citations, creating bibliographies and more. Registration is required.
McDermott Library Tours, ongoing: Did you know that staff can check out up to 50 books at one time and use two streaming movie services on any device? Learn more about those library services and numerous others during a tour. Tours meet at the Information Desk in the Eugene McDermott Library Lobby and last about 30 to 45 minutes. Registration is required.
