Online Consent to Receive Form 1095-C for Calendar Year 2019
In accordance with the Affordable Care Act (ACA) provisions, UT Dallas will have the 2019 Form 1095-C available on or before Jan. 31, 2020. Form 1095-C includes information about the health insurance coverage offered to employees, retirees and their dependents. You will receive Form 1095-C if you were enrolled in the UT Medical Plan or if you worked an average of 30 or more hours per week or were considered a full-time employee in 2019. Graduate student employees who are covered under the UT Student Health Insurance Plan will receive form 1095-B directly from Blue Cross Blue Shield.
If you provide consent, you can access your Form 1095-C online through Galaxy. If you provided consent in 2017 or 2018, you do not have to do so again. The deadline to provide online consent is Jan. 16, 2020. If you do not provide online consent, you will receive the form in the mail on or before the IRS deadline. The forms are expected to be available online on or before Jan. 31, 2020.
If you need to provide online consent, log in to Galaxy with your UT Dallas NetID and password. Then, go to Staff Tools, select ACA Form 1095-C and follow the on-screen instructions. Written instructions and FAQs are posted on the HR website for your reference.

Register for Discounted Employee DART Pass
UT Dallas Parking & Transportation Services and Dallas Area Rapid Transit (DART) are partnering to provide affordable access to public transportation for UT Dallas faculty and staff members who wish to purchase an annual DART pass at the discounted rate of $360 for the 2020 calendar year. The discounted price for employee DART passes will be available until Feb. 14, 2020. After that date, faculty and staff may purchase a DART pass at the DART corporate rate.
To purchase an annual pass, download and complete a Faculty/Staff DART Annual Pass Application and send it to [email protected]. You should receive an email confirmation from our office within one business day. Payment must be made via credit or debit card at the Parking Office located in Parking Structure 3. Applications received after Nov. 21, 2019, may not be processed by DART until after the winter break. To ensure your DART pass is ready by Jan. 1, 2020, you must apply by Thursday, Nov. 21, 2019. Payment must be made by 6 p.m. on Wednesday, Dec.18, 2019.
For 2020, faculty and staff have the option of selecting a DART GoPass App or a GoPass Tap Card. Both options allow for DART Local service. The GoPass App is a digital pass available through the DART GoPass phone app. The GoPass Tap Card is a physical card that requires the user to tap at DART kiosks prior to boarding. The main benefits of the GoPass are that replacements are free since it is registered directly to your cell phone number, and it does not require tapping at the DART kiosks before boarding. Replacement Tap Cards need to be purchased at the prorated rate at the time of reissue or issuance.
Faculty and staff selecting the app option will have their pass automatically activated on their cell phone effective Jan. 1, 2020. Those selecting the card can pick them up at the Parking Office.

University Earns Award for Economic Development
UT Dallas was recently recognized with an Award for Excellence from the University Economic Development Association (UEDA) for showing how the University generated economic development in the region through the creation of the UT Dallas Intellectual Transit-Oriented Transformational Incubator (DITTI).
Dr. Calvin Jamison, vice president for facilities and economic development, and Jenny Mizutowicz, manager of economic development initiatives, accepted the award in the Place & Innovation category at the association’s recent annual summit in Reno, Nevada.
The incubator project provided opportunities for UT Dallas students, faculty and staff to live, learn and connect on campus through the facilities at Northside housing and retail complex developed through a public-private partnership; UTDesign Senior Capstone Program of the Erik Jonsson School of Engineering and Computer Science that partners with industry; and the upcoming DART Silver Line Station.
The project is unique in economic development on a university campus in that it highlights the successful execution of a public-private partnership while also incorporating DART, and creates an intellectual village connected to the world via Dallas/Fort Worth International Airport. It also demonstrates how UT Dallas is dedicated to its strategic initiative of becoming an economic engine for the region.
Established in 1976, UEDA brings together some 160 organizations that include higher education institutions, private sector businesses, non-profits, government organizations, and community economic development stakeholders to create local and regional economic opportunity.

Volunteers Needed for Commencement, Memories on the Mall
Staff members interested in volunteering at the Fall 2019 Commencement Ceremonies and Memories on the Mall are invited to sign up by Friday, Nov. 29. Commencement roles include greeters, ushers and shuttle guides. Duties for Memories on the Mall include distributing alumni tote bags to graduates and serving refreshments. Available shifts include:

Sign Up to Volunteer at Robotics Championship
Volunteers are needed for the 2019 Texas BEST and UIL State Robotics Championship, co-sponsored by UT Dallas.
The deadline for volunteer registration is Tuesday, Nov. 26. The championship will be from Dec. 5-7 at the Comerica Center (formerly Dr Pepper Arena) and Frisco Conference Center. More than 70 teams from all over Texas and the neighboring states are expected to participate, and we need volunteers to donate their time and talent to help these 1,500 students and their coaches, families, and robotics fans.
There are opportunities to virtually assist by reviewing and scoring project notebooks and website design online. Onsite in Frisco, volunteer judges are needed for marketing presentations, table displays, interviews, and more. Game/field support volunteers will be in the pits with all the robotic action, while general volunteers will assist with various tasks. Visit the Texas BEST site to register.

UTD Dietitian Discusses Diet Culture on Podcast
The Office of Communications recently launched the CometCast Network, a series of podcasts produced by UT Dallas staff including alumni.
“Could We Ever…” shines a light on UT Dallas experts and asks them to tackle questions you never knew you needed answered — from science to art and more. New episodes for season one will be released on the first Wednesday of each month through April 2020.
In the latest episode, hosts of the Could We Ever podcast sat down with Sara Asberry, registered dietitian with the UT Dallas Student Wellness Center, to find out if we could ever overcome diet culture.

Donate Hygiene Products, Socks for Those in Need
The Office of Diversity and Community Engagement’s Student Diversity Advisory Council is holding a hygiene product and sock drive to benefit homeless people in the Dallas-Fort Worth area now through November.
Items needed include socks, menstrual products, dental hygiene items, deodorant and new underwear.
Support the cause by dropping off items in collection boxes located by the information desks on the second floors of the Student Union and Student Services Building Addition, spreading awareness of the drive, and sending monetary donations to The Office of Diversity and Community Engagement, 800 W. Campbell Road, AD 26, Richardson, Texas 75080.
