IRS 1095-C Forms Are Now Available
The IRS 1095-C forms (Employer-Provided Health Insurance Offer and Coverage) for 2018 are now available online, if you provided consent for electronic access by the Jan. 13 deadline. All full-time employees, employees who averaged 30 or more hours per week and others enrolled in the UT Select or UT Connect Medical Plan will have a form. It includes information about health insurance coverage offered to employees, retirees and their dependents.
To access the form online, log in to Galaxy and select ACA Form 1095-C under Staff Tools. Once you are in the Tax Form Management, click the menu on the left and scroll to select My Tax Forms. Select the 1095-C for 2018 by clicking the box. Then click the Download button. Please do not click on the Posted Mail button. If you did not provide consent to access the form online, it was mailed to your home address on file on Jan. 22. Forms are being mailed from outside of Texas, so it might take a few days to receive them. Please note that you do not need to include this form to file your taxes. However, you may want to have it for your tax records.
Please do not select prior year forms as there is a charge of $18 for reissues. If you need a prior form, please contact the benefits team.
For more information, email [email protected], contact the employee benefits team, or visit the IRS website or the UT System employee benefits website. |
