New 1095-C Forms Will Now Be Mailed to Employees’ Homes

The new IRS 1095-C forms will be mailed to employees’ home addresses instead of offered online. Technical problems have canceled earlier plans to give UT Dallas employees electronic access to the forms by Jan. 31.
Make sure your mailing address is up-to-date in the PeopleSoft human resources system no later than Friday, Jan. 15. Go to Galaxy and click on Personal Information under Staff Tools to confirm or make changes.
Distribution of the forms will begin around the first week of February. They will go to benefits-eligible employees in accordance with the Affordable Care Act. The form includes information about health insurance coverage offered to employees, retirees and their dependents.
Employees do not need to wait until they receive Form 1095-C before filing their tax returns. Taxpayers comply with minimum essential coverage filing requirements if they simply check a box on their 1040 that they had coverage for the year - they do not need to file any evidence of coverage with their tax forms. The University offers resources to help employees in need of convenient childcare services. Several area providers offer discounts through the Comet Discount Program.
UT Dallas also contracts with Workplace Options, which offers faculty and staff a range of resources and referral services to address dependent care and daily living needs, such as child care, adoption, education, elder care, legal/financial concerns and wellness.
To use Workplace Options, call 866-607-4536 or visit the website through Galaxy. Use the site’s online forms or LiveConnect Web chat feature to be served by a care consultant. If you have general questions about Workplace Options, call the Galerstein Women’s Center at 972-883-6555.

Teaching and Learning Center to Host Scholar Development Event
The recently opened Center for Teaching and Learning will present “Developing Student Scholars: Experiential Learning and Academic Development through Undergraduate Research” noon-1 p.m., Friday, Jan. 22 in GR 4.428. Nominations Sought for University’s Top Student Employees

The Career Center is preparing for National Student Employment Week by asking for nominations for the annual Student Employee of the Year Award.
Work-study and non-work-study student employees are eligible. Nominees must have been employed for at least three months full time or six months part time from June 2015 through May 2016 (anticipated). Research assistants, teaching assistants and students employed under a classification code other than S09996 or S09997 are not eligible for nomination.
Nominations will be accepted through Sunday, Jan. 31. See the Career Center’s National Student Employment Week page for more information. Nomination materials and guidelines are also available online.
The recipient will be announced during National Student Employment Week, April 12-18, when the Career Center honors student employees for their contributions to the University.
Nominations will be judged on reliability, quality of work, initiative, professionalism and the uniqueness of the student’s contributions.

STEM Saturday Activities Discounted for Kids of Employees
The Science and Engineering Education Center offers discounts to children of UT Dallas employees and students who sign up for STEM Saturday programs. The activities, designed for children in grades 2 through 5, are listed on the STEM program page. To qualify for the discount, register with your UT Dallas email address and use the “utdseec” promo code. The fee will be $20 per child (regularly $30 per child).

Registration Open for Live Healthy 10-Week Wellness Challenge
The Live Healthy North Texas 10-Week Wellness Challenge will begin Monday, Jan. 25 and go through April 1. The program encourages participants to form healthy habits through physical activity and improved nutrition. Faculty, staff and students can register online.
Participants work in teams of up to 10 people. To start the registration process, team captains should visit livehealthynorthtexas.org, click “Join Today” and enter LHDUTD for the Group ID to create a team and add team members. Team members will then receive an email with a username and password to log in and complete their profiles.

Presentation to Take a Look at Stress and Compulsive Eating
Faculty and staff are invited to attend a “Stressed is Desserts Backwards” lunch and learn. Presenter Ariel Ketcherside, a doctoral candidate in cognition and neuroscience, will discuss the science of emotion regulation and compulsive eating. The event is set for noon-1 p.m. Wednesday, Jan. 20 in SPN 1.121, in Synergy Park North. RSVP by Jan. 13.

DART’s Fare Deal Returns: Annual Passes on Sale at a Discount
Dallas Area Rapid Transit (DART) and UT Dallas are offering discounts again to faculty and staff members who want to order an annual pass. The passes are valid through Dec. 31.
The discounted passes are on sale for $300 until Friday, Feb. 12, after which the price rises to the DART corporate rate.
