The Student Health Insurance office manages the UT Dallas student health insurance plan, provides confidential claims advising services, and serves as an information resource regarding local healthcare providers. Spring 2014 Waiver Period: Nov. 18, 2013 - Jan. 29, 2014
Spring waiver forms will be available on November 11, 2013. No waiver forms will be accepted until November 18, 2013.
International students, unless you apply for or are granted a waiver, student health insurance will be charged to your student account. If you have any questions about the charge, please email firstname.lastname@example.org or call the student health center at 972-883-2747.
The University of Texas System is pleased to offer a new Student Health Insurance Plan underwritten by Blue Cross and Blue Shield of Texas. The plan is administered by Academic Health Plans for the 2013-2014 school year. This student-focused plan protects students at school, at home, and while traveling or studying abroad. We now have two different waiver forms depending on the type of insurance that you have. If you have an insurance policy through an employer, please complete the Employer Waiver Form. If you have any other type of insurance, please complete the Private Waiver Form. Please contact us if you have any questions.
Interim Coverage Letter for International Students
Because your student health insurance coverage must be renewed each semester, the dates of your coverage coincide with each respective semester. Should you need to receive medical treatment or fill a prescription between the start date of the current semester’s insurance policy and census day of the current semester, please contact email@example.com or call 972-883-2747 to secure an Interim Coverage Letter. The Interim Coverage Letter will serve as your temporary proof of insurance until the insurance company is able to mail your official insurance card, which will be sometime after census day.If you need to add dependents or extend dependent coverage on your policy, you will need to request that your coverage be updated directly with the insurance company. Dependent enrollment cannot be processed until the student’s enrollment has been updated to reflect the current semester’s coverage.
