A.  Physical Check

1.  Case: Clean and intact (loose screws, broken parts, cracks?)
2.  Earmold: Cerumen, dirt, cracks?
3.  Tubing: Clean, cracked, crimped, clogged, collapsed, connected to earmold/aid securely.

B.  Battery Check

4.  Clean: Free of corrosion/dirt etc.?
5.  Charge: In green(good) range or above 1.0 volts on tester?
6.  Size: Correct size to fit battery compartment?
7.  Compartment/contacts: Clean and intact?
8.  Insertion: Proper placement – "+" and "-" sides oriented properly?

C.  Listening Check

9.  Switch: Off on "O", amplification on "M", Hum on "T"?
10.  Noise: Unwanted sounds – hum, static, motorboating?
11.  Feedback: Squealing at low to moderate volume settings or at high settings with earhook covered?
12.  Linearity of gain: Volume increases gradually as volume control is turned up?
13.  Scratchiness: As volume control is adjusted?
14.  Intermittence: Reduction in gain or cutting out as aid is pressed between fingers? As controls are manipulated?
15.  Distortion: Loss of clarity with 6 sound test (u, i, a, s, sh, m)?
